K-5 Faith Formation

​Faith Formation for Grades K-5 takes place from 9:30 am to 10:45 am Sundays during the school year at the Faith Formation Center.  To receive more information, contact Sr. Jeanne Marie in the Parish Office.​

2024 – 2025 Registration

We are accepting registrations for K-5 Faith Formation for the upcoming 2024 – 2025 Catechetical Year. Second Graders must fill out an additional Sacramental Preparation form for First Reconciliation and First Communion.

Please register electronically by following these steps:

  1. Download the Registration Form (.pdf) to your computer.
  2. FILL IN the form in your PDF viewer program (e.g. Adobe Acrobat Reader).
  3. SAVE the completed form.
  4. EMAIL the completed form as an attachment to Sr. Jeanne Marie

Second Grade Families:

Please also download, fill in, save, and email the Form for Second Grade Reconciliation and First Eucharist (.pdf) for each child who will be receiving Sacraments this year.
